Digital Management Group:Study participants received daily diabetes-related education during hospitalization and were instructed to use the digital platform (basic version) to self-learn about diabete
after discharge, they were remotely managed by the digital platform (upgraded version) and followed up regularly.
Control group:Study participants received daily diabetes-related education during hospitalization and were instructed to use the digital platform (basic version) for self-learning about diabetes
after discharge, they were followed up by telephone (first week, first month, third month, and sixth month) and provided health education.

Inclusion criteria
Inclusion criteria: (1)Patients with diabetes diagnosed according to the 2022 ADA diabetes diagnostic criteria; (2)Age = 18 years, no verbal communication difficulties (3)Have a smartphone (Android or iOS); (4)Voluntary participation in this study.
Exclusion criteria
Exclusion criteria: (1)Mental illness or cognitive impairment and dysfunction; (2)Suffering from severe cardiac, renal or cerebrovascular disease or cancer, with a life expectancy of no more than one year.
