12 Years to 25 Years

Inclusion criteria

Inclusion criteria: (1) Age between 12 and 25 years (including the threshold); (2) Diagnosed with chronic kidney disease according to the KDIGO guidelines with a duration of more than 3 months; (3) Be conscious and have normal reading, writing, communication and expression skills; (4) Stable for at least one month if on psychotropic medication; (5) Adolescents (minors also need their guardians) gave informed consent to voluntarily participate in this study.

Exclusion criteria

Exclusion criteria: (1) Critical illness or a combination of serious complications; (2) Diagnosed with moderate to severe cognitive impairment or other psychological or psychiatric problems requiring immediate attention; (3) Inability to cooperate with the full investigation; (4) Have been studied in the past in other transition-ready related interventions.
