Inclusion criteria
Inclusion criteria: Hypertensive patient inclusion criteria a. Age: 18 years or above b. Diagnosed to have hypertension c. Regularly followed up at the designated community centre for the previous year, as indicated by having previous regular follow up appointments, and regular prescriptions of hypertension-related medications at the centre. Diabetic patient inclusion criteria a. Age: 18 years or above b. Diagnosed to have diabetes c. Regularly followed up at the designated community centre for the previous year, as indicated by having previous regular follow up appointments, and regular prescriptions of diabetes-related medications at the centre.
Exclusion criteria
Exclusion criteria: Participants who don't pass the inclusion criteria.
