Inclusion criteria
Inclusion criteria: The investigated patients aged 45 to 75 with type 2 diabetes who had poor control of blood sugar and weight in 2019 China diabetes Complications Research in Sichuan Province Qingyang District. At the time of the survey, it was still under the management of the community medical service center in the jurisdiction, and its management community could provide physical examination information (weight, blood pressure, fasting blood glucose, blood lipids, glycosylated hemoglobin, renal function measurement, etc.) and fundus lesions information for follow-up within three months. Have certain cognitive abilities and can independently complete relevant questionnaires.
Exclusion criteria
Exclusion criteria: Individuals with contraindications to physical activity, a previous history of mental illness, unwilling to participate in this study.
