Inclusion criteria
Inclusion criteria: ?The permanent residents aged = 60 years old in Lanzhou. ? in line with the diagnostic criteria of the ' Chinese Guidelines for the Prevention and Treatment of Type 2 Diabetes ( 2020 Edition ) ', ? in line with the diagnostic criteria of the ' Chinese Guidelines for the Prevention and Treatment of Hypertension ( 2018 Revision ) '. ?No children and nannies to take care of, husband and wife live together or live alone. ?clear consciousness, have a certain ability to read and write, no communication barriers. ?voluntarily participate in the project.
Exclusion criteria
Exclusion criteria: ? being or recently participating in other related researchers. ? serious complications, leading to communication disorders, activity disorders, unable to care for oneself. ?have a clear history of mental illness.
