75 Years to 105 Years
Inclusion criteria
Inclusion criteria: 1) Lumbar and epidural anesthesia femur, tibia and fibula fracture, hip and knee replacement surgery 2) Combined epidural anesthesia 3) =75 years old 4) ASAI-III level 5) BMI1h 7) Length of hospital stay>72h 8) Postoperative unplanned sedation and unplanned admission to ICU
Exclusion criteria
Exclusion criteria: 1) Cognitive Summary Mental State Test (MMSE) score 6h, drug-induced delirium, alcohol withdrawal delirium. 3) Dementia, Parkinson's disease, delirium, seizure, stroke within 1 month, intracranial disease or intracranial surgery within 1 year. 4) Severe audio-visual impairment, mental illness. 5) Use atypical antipsychotics, cholinesterase related drugs, dopaminergics, benzodiazepines, steroids and melatonin drugs for a long time or nearly a week. 6) Intraoperative atropine dosage>2mg. 7) Use of postoperative delirium cluster nursing program. 8) Evidence of spinal injury, nerve damage, 9) Diabetes that requires insulin control, 10) Preoperative electrolyte disturbance, systemic inflammatory response syndrome, cancer, hemodialysis treatment within a week 11) Hypotension (mean arterial pressure100ml/ day) 13) There are contraindications or allergies to the drug in this study.
