interventional group:On the basis of routine health consultation in the control group, the pre-constructed hierarchical care training program for AD patients was included for training.

Inclusion criteria

Inclusion criteria: Inclusion criteria : 1 patients aged 60 years and above ; 2 Patients were diagnosed as AD according to the diagnostic criteria of WHO International Classification of Diseases ( ICD-10 ). Caregiver inclusion criteria : 1 age = 18 years old ; 2 Primary school and above culture, with a certain reading, understanding and communication skills, can use smart phones ; ( 3 ) Main caregivers of AD patients : family caregivers ( including spouses, children or other close relatives ) with continuous home care time > 3 months and weekly care days > 5 days ; 4 All caregivers were informed of the study and signed the ' informed consent form ' ; 5 no major physical illness and mental illness ;

Exclusion criteria

Exclusion criteria: Exclusion criteria : 1 End-stage patients with severe, critical or unstable disease. Exclusion criteria : 1 Patients with severe diseases such as severe organ damage, cardiovascular and cerebrovascular diseases need emergency hospitalization during the intervention period. Exclusion criteria for caregivers : 1 Have participated in or are participating in similar care training studies ; 2 care remuneration ; 3 Volunteers of public service ; 4 Caregivers will not be provided in the foreseeable future ( within the study period ). Caregiver exclusion criteria : 1 Caregivers could not be contacted for reasons such as termination or relocation during the study ;
