Interventions

Control Group:The subjects will receive regular routine care at the community hospital, with no changes to the existing standard care model, except for regular follow-up conducted by the community man

Intervention Group:Beyond the diagnostic and treatment services and thematic diabetes education similar to those provided to the control group, the intervention group will receive additional intervent

Inclusion criteria

Inclusion criteria: ?aged = 18 years; ?diagnosed with type 2 diabetes according to the diagnostic criteria outlined in the clinical guideline of the Chinese Diabetes Society; ?have at least one complete outpatient or inpatient medical record related to type 2 diabetes; ?willing to sign an informed consent form and participate in follow-up. ?capable of cooperating with the diabetes management interventions.

Exclusion criteria

Exclusion criteria: ?severe liver or kidney dysfunction; ?impaired consciousness or inability to communicate effectively; ?failure to sign an informed consent form; ?significant organ failure, severe coronary artery disease, cardiovascular diseases, or a history of myocardial infarction within the past 12 months prior to enrollment; ?history of severe neurological or psychiatric disorders; ?severe infections; ?active disseminated intravascular coagulation (DIC); ?malignant tumors; ?alcohol or substance abuse disorders; ?pregnant or breastfeeding women.
