Control group:For routine discharge nursing, the responsible nurse shall do health education, medication guidance, rehabilitation exercise, safety, etc., the day before discharge.
The experimental group:on the basis of the control group, after the patient was discharged, the health education guidance of related diseases was pushed on the Wechat platform.

Inclusion criteria
Inclusion criteria: (1) Stroke patients diagnosed by CT or MRI. (2) left limb dysfunction, muscle strength less than grade 5 (3) the vital signs are stable. (4) patients or their families have the ability to use smartphones to acquire rehabilitation training knowledge.
Exclusion criteria
Exclusion criteria: 1) limb dysfunction before stroke (2) unconsciousness or mental disorder (3) complicated with heart, lung, kidney and other organ injuries. (4) No family members or family members had abnormal cognition (unable to communicate normally) and abnormal mental behavior.
