Inclusion criteria
Inclusion criteria: Those who voluntarily participate in the investigation, follow-up and receive relevant medical services, and Aged 65 and above, and The health condition is stable, or the patient is in a stable period of chronic disease (the patient does not need to be hospitalized or go to the hospital temporarily in the near future), and Permanent residents of Chaoyang District, Beijing On the basis of meeting the four criteria for inclusion of research objects, FRAIL weakness score is 1 point or more, or ADL score is not full (=2 points). Can be recruited as participants.
Exclusion criteria
Exclusion criteria: Participants with severe loss of physical or mental functions (need long-term care, ADL=2), are excluded from the study.
