Inclusion criteria

Inclusion criteria: ?Patients with primary hypertension (diagnosed according to the diagnostic criteria outlined in the "Chinese Guidelines for the Prevention and Treatment of Hypertension"). ?Patients who were enrolled in the follow-up management information system between January 1, 2011, and December 31, 2019, and were still under follow-up management in the community at the initiation of the intervention, with scheduled follow-ups in the same community within the next year. ?Patients without organic complications such as cardiovascular, cerebral, renal, or ocular disorders prior to the community intervention study. ?High-risk population for complications: Identified through a rapid screening method and randomly selected from the population, comprising individuals who have three or more key risk factors out of the 13 defined key risk factors. The key risk factors include poorly controlled blood pressure, high salt intake, overweight/obesity, irregular medication adherence, age =60 years, smoking, excessive alcohol consumption, family history of cardiovascular diseases, abdominal obesity, long disease duration, abnormal blood lipid levels, lack of physical exercise, and high psychological stress. ?Age between 35 and 80 years. ?Normal cognitive function, hearing, and intelligence, with a certain level of communication and expression ability. ?Voluntary participation and informed consent to participate in this study.

Exclusion criteria

Exclusion criteria: ?Patients with secondary hypertension. ?Patients with coexisting malignant tumors or diabetes. ?Patients with a history of relevant diseases such as cardiovascular, cerebral, renal, or ocular disorders. ?Patients with mental illnesses.
