Inclusion criteria
Inclusion criteria: The research object of this project is T2DM patients with inadequate blood sugar control: (1) The subject was diagnosed with type 2 diabetes (according to the Chinese Guidelines for the Prevention and Treatment of Type 2 diabetes, 2020) (2) Men and women aged = 18 years old (3) Poor blood sugar control in the past six months (HbA1c>7.0%) (4) Subject is able to use the internet and mobile intelligent devices (5) Having complete cognitive and behavioral abilities (6) The patient is willing and able to sign an informed consent form before participating in this study
Exclusion criteria
Exclusion criteria: (1) Patients with language communication, cognitive impairment, and severe mental illness (2) Participating in or having participated in any other clinical trials within the past 3 months (3) Planned migration, unable to continuously follow up at this research center (4) Pregnant and lactating women (5) Individuals with clear systemic diseases, including (but not limited to):ASCVD (coronary heart disease, ischemic stroke, peripheral artery disease) acute phase events or heart failure, Pulmonary embolism; Hematological diseases such as anemia; Patients with abnormal renal function: blood creatinine exceeds the upper limit of normal value; Obvious abnormal liver function: Transaminase and/or total bilirubin = 2 times the upper limit of normal; Rheumatic system diseases: those who are being treated with glucocorticoid and Immunosuppressive drug; Respiratory disease: patients with serious lung disease who are currently treated with oral glucocorticoid; Individuals with abnormal endocrine function, such as pituitary dysfunction; HIV positive (self reported); Cancer: Cancer patients who have received treatment within the past 5 years; Organ transplant patients; Other situations where the researcher deems it inappropriate to participate in this study, such as drug use, alcohol abuse, and substance abuse.
