60 Years to 100 Years

Inclusion criteria

Inclusion criteria: ? Comply with the diagnostic criteria for heart failure patients in the 2018 Chinese Guidelines for the Diagnosis and Treatment of Heart Failure upon admission; ? Heart failure as the main admission diagnosis; ? Age = 60 years old; ? Ability to read and express oneself in language, with no communication barriers.

Exclusion criteria

Exclusion criteria: ? have experienced myocardial infarction, stroke, cancer, etc. in the past 3 months (combined with the aforementioned diseases, which seriously affect the clinical outcomes of heart failure patients; ? plan to enter a care institution after discharge; ? have participated in other intervention trials besides routine management in the past 3 months.
