Interventions

Intervention group - HCHW:Participants will receive the same usual care as participant in the control group (see above). The health coaching session will be conducted by health coaches with a bachelor

Inclusion criteria

Inclusion criteria: (i) documented newly diagnosed HT cases (diagnosed less than 6 months when we contact them) irrespective to whether there are on medication or not (ii) age 18 or above years old, (iii) able to provide informed consent.

Exclusion criteria

Exclusion criteria: (i) history of dementia or mental illnesses that may become barrier to the health coaching; (ii) do not have an active mobile phone to synchronize with the health wearable, (iii) not sufficient Cantonese language proficiency to read text messages or having oral conversation with a health coach, (iv) with end-stage disease.
