Inclusion criteria
Inclusion criteria: ? =18 years of age; (2) Patients diagnosed with hypertension according to the diagnostic criteria of hypertension in the Chinese Guidelines for Hypertension Prevention and Treatment (2018 Revision) ; (3) Admitted patients diagnosed with AMI according to the diagnostic criteria of AMI in the Fourth Edition of the General Definition of Myocardial Infarction (2018); (4) Have a smart phone and can use wechat for online communication; (5) Patients and their family members agreed to participate in the study voluntarily.
Exclusion criteria
Exclusion criteria: ? Patients with severe or above heart failure (Killip ? and ?), or complicated with cerebral infarction, tumor and other serious diseases; (2) Karnofsky functional status score was dependent type (KPS < 50 points), indicating patients unable to take care of themselves; ? There is a history of cognitive dysfunction, confusion, or mental illness.
