Inclusion criteria
Inclusion criteria: 1. Age =45 years old; 2. Type 2 diabetes mellitus, hypertension, atrial fibrillation (if two or more can not meet the third point); 3. Dyslipidaemia; Smoking history; Overweight or obese; Family history of stroke or coronary heart disease with two or more risk factors; 4. Voluntary participation and signed informed consent.
Exclusion criteria
Exclusion criteria: 1. Due to any of the following circumstances, both upper limbs cannot use the watch to collect information: (1) Both upper limbs are disabled; (2) Abnormal wrist skin; (3) severe occlusive vascular disease of the upper limb; (4) significant edema of upper limb; 2. Type 1 diabetes mellitus; 3. Allergic to watch/bracelet devices; 4. Patients with stroke and coronary heart disease; 5. Unable to self-measure blood pressure and blood sugar; 6. Severe mental disorder, unable to express wishes, and unable to appoint guardians and caregivers; 7. Those who have participated in other health management projects; 8. Researchers judged that long-term interviewers could not be completed.
