Age

60 Years to 100 Years

Inclusion criteria

Inclusion criteria: 1. Has been diagnosed with at least one chronic disease by a second level or higher hospital; 2. Age = 60 years old; 3. After discharge, use at least one medication for treatment; 4. After discharge and returning to home; 5. The discharge time is less than 2 months; 6. Participate in this study with informed consent.

Exclusion criteria

Exclusion criteria: 1. Those who need to be transferred to other medical institutions for further treatment due to their illness after discharge; 2. Critical and/or end-stage patients with easily changing conditions; 3. Those who cannot take care of themselves; 4. Individuals with hearing, visual impairment, or communication difficulties with others.
