Inclusion criteria
Inclusion criteria: a. Organization: Inclusion criteria: ? The institution is located within the administrative scope of Guizhou Province; (2) To carry out chronic disease diagnosis and treatment and health management services for diabetic patients; ? Health examination services can be carried out. b. Head of the organization: Inclusion criteria: ? belong to the organization's staff, contract system or establishment; (2) Familiar with the procedures of diabetes diagnosis and treatment in institutions; ? Must have been employed for more than one year. c. Doctor: Inclusion criteria: ? Have a doctor's certificate; (2) Participate in the diagnosis, treatment and health management of diabetic patients. e. Public health personnel: Inclusion criteria: ? Participate in the diagnosis, treatment and health management of diabetic patients. f. Patient Inclusion criteria: ? Meeting the national diagnostic criteria for type II diabetes; ? Live in the community where the local primary medical institutions are located; ? Age =35 years old.
