Inclusion criteria

Inclusion criteria: Patients' health condition: • =18 years or older • Diagnosed with advanced cancer • Within 1 year life expectancy estimated by clinicians • Able to nominate a willing caregiver who would involve in their medical care discussion Family caregivers: • =18 years or older • Potential surrogate decision-maker nominated by patients; or Primary medical decision-maker for patients; • Able to communicate in Mandarin and read in the simplified Chinese • Able to give consent

Exclusion criteria

Exclusion criteria: Caregivers will be ineligible if they are: a. Presence of any impairments influencing communication and decision-making capacity identified (e.g. advanced dementia, deafness or aphasia).
