Inclusion criteria

Inclusion criteria: Age=18 years old uncontrolled hypertension patients with CVD willing to participate and sign the consent form

Exclusion criteria

Exclusion criteria: secondary hypertension serious kidney disease(CKD 4 or higher ) pregnant or plan to be pregnant moveout the community within 12 months life expectancy less than 12 months and cannot walk to clinics
