Inclusion criteria: (1) Age >=18 years old, informed consent; (2) clinically diagnosed chronic disease; (3) Have certain reading and understanding ability; (4) Have smart phone, can use flexibly; (5) Live in Nantong urban area;

Exclusion criteria

Exclusion criteria: (1) Refuse to participate; (2) Severe cognitive dysfunction (MMSE<=9); (3) Can't use smart phone; (4) intensive care patients;
