intervention group:Threshold load inspiratory muscle training and Routine pulmonary rehabilitation
control group :Routine pulmonary rehabilitation, including airway clearance, physical activity, and postural management.

Inclusion criteria

Inclusion criteria: 1. Patients with neuromuscular disease with tracheostomy and mechanical ventilation for more than 48 hours 2. Patients with neuromuscular diseases who have been assessed by clinicians as being stable and ready to be weaned, and who cannot tolerate weaning within 24 hours

Exclusion criteria

Exclusion criteria: 1. Patients who are unconscious (GCS score less than or equal to 8 points) and unable to cooperate 2. Patients with other respiratory and circulatory diseases, including COPD, chronic heart failure, pneumothorax, severe emphysema, pulmonary hypertension, uncontrolled asthma, etc.
