Inclusion criteria
Inclusion criteria: 1. Patients with T2DM meeting the 2019 WHO diabetes diagnosis and classification criteria; 2. the latest HbA1c test result of 7%=HbA1c=10% 3. Time from diagnosis =1 year; 4. Local permanent resident population; 5. Age=18 years: 6. With normal cognition and communication skills, able to understand and accept the guidance and evaluation of chronic disease management; 7. Capable of using a smartphone; 8. Willing to participate in the study and sign the informed consent form.
Exclusion criteria
Exclusion criteria: 1. Type 1 diabetes: 2. Pregnancy or planned to be pregnant within six months; 3. Insulin pump users; 4. Frequent medication adjustment needed due to unstable condition in the first three months; 5. With severe diabetic complications or obvious organ dysfunction; 6. History of alcohol or drug abuse; 7. Unable to operate mobile APP due to the ability of reading and cognition; 8. Patients with mental disorders who can not take care of themselves; 9. Patients with tumors who have received radiotherapy or chemotherapy in the past six months; 10. Patients who have participated in other relevant intensive lifestyle intervention studies in the past six months; 11. Patients who are participating in other research or health management projects.
