Inclusion criteria

Inclusion criteria: ? Patients with stage 4-5 who meet the diagnostic criteria for CKD, eGFR<30 ml·min-1· (1.73 m2)-1 ? Age? 18 years old, able to take care of themselves or have care; ? The patient's or caregiver has normal cognitive function; ? Those who voluntarily participate in this research and sign the informed consent form.

Exclusion criteria

Exclusion criteria: ? Have received kidney transplantation or have started dialysis treatment; ? Those who can only undergo a specific dialysis treatment due to their health conditions; ? Those who cannot follow up on time.
