cardiogenic shock

Interventions

Haemodynamic monitoring group:1. Head high, foot low position 15 degrees for 1 minute (baseline 1), recording baseline haemodynamic parameters
2. Trendelenburg position: for 3 minutes, recording haemodynamic parameters
3. Return to initial position (baseline 2) and record haemodynamic parameters
4. Perform volume loading test (NS 500ml infusion for 30 minutes) and record haemodynamic parameters.

18 Years to 80 Years

Inclusion criteria

Inclusion criteria: 1. Receiving VA-ECMO support 2. ECMO flow rate: 2-3L/min 3. Receiving invasive mechanical ventilation 4. Require fluid therapy which was decided by ICU physician to (hypotension, hypoperfusion or attempt to reduce vasoactive drugs)

Exclusion criteria

Exclusion criteria: 1. Age less than 18 years 2. Pregnant or planning to become pregnant, and breastfeeding women 3. Pulseless (pulse pressure difference less than 15 mmHg) 4. moderate to severe aortic valve insufficiency 5. Contraindications to Trendelenburg (abdominal hypertension, gastric retention, etc.) 6. Active haemorrhage 7. Cardiac arrhythmia 8. Unstable ECMO flow 9. Poor ultrasound translucency 10. Participant in other clinical trials
