Inclusion criteria

Inclusion criteria: 1. Age> 18 years old 2. Community visits meet the diagnostic criteria for CKD, including: Estimated glomerular filtration rate based on serum creatinine-based CKD-EPI 30 mg/g 3. High-risk groups for CKD, including the following groups Diabetes, hypertension, cardiovascular disease, previous history of AKI, family history of kidney disease, kidney stones, obesity, advanced age (> 60 years); 4. The number of community medical visits is more than 2 times per year

Exclusion criteria

Exclusion criteria: 1. Pregnancy 2. Maintenance renal replacement therapy population 3. People who have less than 2 community medical visits per year and cannot regularly visit the community 4. Non-resident social security groups in Beijing

Cardiovascular disease event rate (coronary atherosclerotic heart disease, arrhythmia, non-fatal myocardial infarction, non-fatal stroke, heart failure, coronary heart disease-related surgical procedures, death from cardiovascular disease);Hospitalization rates for renal events in people at high risk of chronic kidney disease;
