Inclusion criteria
Inclusion criteria: 1. The diagnosis of stroke is confirmed by cranial CT or MRI and meets the diagnostic criteria of the Chinese Diagnostic Guidelines for Major Cerebrovascular Diseases 2019. 2. First presentation, no previous history of stroke, age 18-80 years, duration of disease = 6 months. 3. Visual analogue score (VAS) of =4 for pain during passive abduction or external rotation of the shoulder joint 4. No drug injection treatment of the shoulder joint within 6 months 5. Modified Ashworth Spasticity Scale rating between 0 and I+ during shoulder external rotation or abduction 6. ultrasound of the shoulder joint: no concomitant subacromial deltoid bursitis or biceps tendonitis 7. no other osteoarthritic muscle disease or other neurological disorders that significantly affect motor function 8. no obvious communication and comprehension difficulties, able to communicate normally and cooperate in completing the experiment 9. stable vital signs and able to cooperate with the examination
Exclusion criteria
Exclusion criteria: 1. allergy to botulinum toxin or glucocorticoids 2. intra-articular glucocorticoid or botulinum toxin injections in the shoulder joint within the last 6 months 3. have a previous history of shoulder surgery, frozen shoulder, history of rotator cuff injury, history of trauma to the shoulder joint, etc. 4. have other conditions that are more painful than hemiplegic shoulder pain 5. having an active malignancy or other life-threatening disease 6. have a blood clotting disorder, diabetes, gastric/duodenal ulcer, infection (within the last 6 months) 7. current use of oral glucocorticoids, non-steroidal anti-inflammatory drugs or immunosuppressive drugs. 8. pregnant and lactating females. 9. those who have not signed the informed consent form
