Inclusion criteria
Inclusion criteria: 1. Aged 45-60 years; 2. Chronic disease patients who have met the national diagnostic criteria for hypertension and chronic disease at level I or above through three blood pressure or ambulatory blood pressure tests (according to the 2018 revision of the Chinese Hypertension Prevention and Treatment Guidelines) and who do not have complex complex complex chronic diseases and enjoy national medical insurance subsidies; 3. The condition is relatively stable; 4. Have clear awareness and compliance behavior; 5. Have some experience in using intelligent mobile devices; 6. The patient voluntarily participates and signs an informed consent form. Only those who meet the above conditions can be included.
Exclusion criteria
Exclusion criteria: 1. Accompanied by comprehensive diseases such as severe illness, brain, kidney, or serious complications 2. Accompanied by progressive damage to organs or blood vessels; 3. Lack of self-care ability or significant cognitive, language and hearing impairment; 4. Other mental diseases; 5. The family members have obvious objections. Any one of the above is discharged.
