Inclusion criteria
Inclusion criteria: Aged>18 years, operative time >2h.At increased risk of postoperative complications, defined as least one of the following criteria: 1,Age =70 years.2,Known or documented history of coronary artery disease.3,Known or documented history of heart failure.4,Diabetes currently treated with or oral hypoglycemic agent and/or insulin.5,Morbid obesity(BMI=35kg/m2).6,Preoperative serum albumin<30g/l.7,Or two or more of the following risk factors: a, ASA 3 or 4.b, chronic respiratory disease. c, obesity (BMI 30-35kg/m2). d, aortic or peripheral vascular disease. e,preoperative hemoglobin<100g/L.
Exclusion criteria
Exclusion criteria: 1. Urgent or time-critical surgery. 2. ASA physical status 5-such patient are not expected to survive with or without surgery, and their underlying illness is expected to have an overwhelming effect on outcome(irrespective of fluid therapy) 3. Preoperative serum creatinine>200umol/l(2.8mg/dl)or chronic renal failure requiring dialysis
