Inclusion criteria
Inclusion criteria: 1. Experimental group and control group (elderly patients): (1) Elderly people aged >= 60 years; (2) Elderly people with chronic diseases, including common elderly chronic diseases such as cardiovascular system, respiratory system, nervous system, digestive system and urinary system; (3) Living in the community for 1 year or more; the elderly have informed consent and can communicate normally; (4) Can get in touch with the children of the elderly, and the children are willing to participate in this research; 2. Modeling group (elderly patients): (1) Elderly people aged >= 60 years; (2) Elderly people with chronic diseases, including common elderly chronic diseases such as cardiovascular system, respiratory system, nervous system, digestive system and urinary system; (3) The elderly have informed consent and can communicate normally; 3. Interview group: (1) Elderly patients: 1. Have at least one chronic disease; 2. The course of disease is half a year or more; 3. Age >= 60 years old; 4. Good cognitive and mental state; 5. Stable condition; 6. Informed consent and voluntary participation in this study; (2) Family members: 1. The main caregiver of the patients included in the study; 2. Informed consent and voluntary participation in this study; (3) Medical staff: 1. Have more contact with patients included in the study, provide health education, health testing and guide self-management of elderly patients and their families; 2. Informed consent and voluntary participation in this study.
Exclusion criteria
Exclusion criteria: 1. Experimental group and control group (elderly patients): (1) Those who have been clearly diagnosed by a doctor as suffering from mental illness and severe disabilities; (2) Temporary resident population in the community (including relocation of residence due to medical treatment); 2. Modeling group (elderly patients): those who are clearly diagnosed by doctors as suffering from mental illness and severe disabilities. Those who have been clearly diagnosed by a doctor as suffering from mental illness and severe disabilities; 3. Interview group: (1) Elderly patients: 1) Serious physical illness; 2) There is an acute attack of a chronic disease; 3) There is a situation that affects communication; (2) Family members: did not provide financial or emotional support or life care for elderly patients on weekdays; (3) Medical staff: 1) Being on probation; 2) Failure to register for practice; 3) The working hours are within half a year.
