Inclusion criteria

Inclusion criteria: 1. At least 18 years old; 2. Patients treated with intravenous vancomycin; 3. Receive renal replacement therapy.

Exclusion criteria

Exclusion criteria: 1. Pregnant women; 2. Patients who interrupt renal replacement therapy due to clinical or technical reasons; 3. Patients who cannot obtain written informed consent; 4. Patients died within 24 hours after admission; 5. Other patients to be excluded.
