Inclusion criteria

Inclusion criteria: 1. Patients diagnosed with COPD by a respiratory specialist; 2. Aged >=40 years; 3. Informed consent and voluntary participation in this study.

Exclusion criteria

Exclusion criteria: 1. Participated in other self-management plans within the past 6 months; 2. Suffering from major physical diseases, such as malignant tumors; 3. Have language communication disorders, consciousness disorders or serious mental disorders.
