Inclusion criteria
Inclusion criteria: 1. Diagnostic criteria for type 2 diabetes mellitus: fasting blood glucose >=7.0mmol/L or OGTT2 hour blood glucose >=11.1mmol/L or HbA1C>=6.5%; 2. Identification of high-risk groups of diabetes: According to the National Guidelines for Prevention and Management of Grassroots Diabetes (2018), 12 risk factors can be considered when defining high-risk groups of type 2 diabetes. Based on the accessibility of physical examination and screening data, this study selected several major risk factors for screening high-risk groups: (1) Aged >=40 years; (2) A history of prediabetes (IGT, IFG, or both); (3) Overweight (BMI>=24 kg/m^2) or obese (BMI>=28 kg/m^2) and/or central obesity (waist circumference >=90 cm for men and >=85 cm for women); (4) Sedentary lifestyle; (5) Family history of type 2 diabetes in first-degree relatives; (6) Women with a history of gestational diabetes; (7) Hypertension (systolic blood pressure >=140 mmHg and/or diastolic blood pressure >=90 mmHg), or being treated for blood pressure reduction; (8) Abnormal blood fat (HDL-C=2.22mmol/L), or receiving lipid regulation therapy; 3. Aged 18-75 years (note: diabetes risk group >=40 years old); 4. Permanent residents of Beijing (annual travel time less than 1 month); 5. Did not take any psychotropic drugs before enrollment; 6. Able to use smart phones and familiar with wechat functions; 7. Volunteer to participate in the study and fill in the informed consent form.
Exclusion criteria
Exclusion criteria: 1. Complicated with severe heart, brain, kidney, eye, foot and nervous system complications (proliferative retinopathy, nephropathy stage IV or above or creatinine >2mg/dl, cardiac function grade III or below, cerebrovascular accident sequelae, diabetic foot grade I or above); 2. Patients with mobility difficulties, confusion and abnormal spirit; 3. Patients suffering from tumor and receiving radiotherapy or chemotherapy within half a year; 4. Participating in other similar research projects; 5. Uncooperative.
