Inclusion criteria

Inclusion criteria: 1. Adult patients aged 18 years or above; 2. Residents of the North District villages, the presence of diabetes or hypertension who are patients of the North District GOPCs and who are in stable condition.

Exclusion criteria

Exclusion criteria: Those who are non-ambulatory.
