18 Years to 90 Years

Inclusion criteria

Inclusion criteria: (1) With a definite diagnosis of chronic heart failure; (2) Aged = 18; (3) Screened by the Tampa Scale for Kinesiophobia-Heart and with a score =38points; (4) Own and be able to use Intelligent mobile phone and use WeChat; (5) Be with clear conscious, can fluently expressive by language, and be able to cooperate to complete the intervention study; (6) Voluntary participation in this study

Exclusion criteria

Exclusion criteria: (1) With cognitive impairment or serious mental illness (2) be severe heart failure that need absolute bed rest (3) With severe physical activity disorder or combined with other major diseases that lead to immobility (4) with severe hearing and visual impairment who cannot communicate (5) Unable to understand the intervention content and cannot complete the study
