Inclusion criteria
Inclusion criteria: (1) Age =65 years old; (2) have lived in the community for at least 1 year; (3) The patient was diagnosed with stroke by head CT or MRI (meeting the diagnostic criteria for stroke formulated by the fourth Conference on Cerebrovascular Diseases); (4) No communication barriers, can answer correctly, can fill in the questionnaire independently or with the help of researchers; (5) no mental or cognitive impairment; (6) Primary school education level or above, normal reading comprehension; (7) Able to communicate effectively by telephone; (8) can use wechat or can use wechat after education; (9) Informed consent.
Exclusion criteria
Exclusion criteria: (1) Myocardial infarction and angina attack in the past year; (2) severe heart, liver and kidney diseases; (3) excessive physical weakness and inability to tolerate muscle strength tests; (4) Patients with hypertension whose blood pressure is higher than the normal range and cannot be controlled by drugs; (5) Patients with history of syncope after exercise; (6) Mental and cognitive disorders.
