Inclusion criteria

Inclusion criteria: 1. Aged >=18 years; 2. Meet the diagnostic criteria for heart failure and the New York Heart Association heart function I-IV rating; 3. Able to complete the questionnaire; 4. Able to communicate normally in Mandarin or Cantonese; 5. Can use wechat or mobile phone for follow-up; 6. Agree to participate in the study.

Exclusion criteria

Exclusion criteria: 1. Acute heart failure; 2. Suffer from cognitive impairment, such as dementia; 3. Suffer from mental illness; 4. Suffering from other serious diseases, such as kidney failure, terminal cancer, etc.; 5. Recently participated in other research projects.
