Inclusion criteria
Inclusion criteria: 1. Patients: (1) diagnosed with chronic heart failure, heart function class I-III; (2) Age >= 60 years; (3) Informed consent and signed a paper version of the informed consent form; (4) Normal cognitive function, able to communicate and exchange normally; (5) Have some reading and comprehension ability, able to use smart phones. 2. Caregivers: (1) Family members of the patient and aged >= 18 years; (2) Informed consent and signed the paper version of the informed consent form; (3) Taking care of the patient for most of the time >= 8 hours per day and >= 5 days per week; (4) Normal cognitive function, able to communicate and exchange normally; (5) having some reading and comprehension ability and able to use smart phones.
Exclusion criteria
Exclusion criteria: 1. Patients: (1) Participating in other studies or trained in heart failure care; (2) Leaving Wuxi within six months; (3) With other serious life-threatening diseases, such as malignancy, advanced liver and kidney disease, leukemia, etc.; (4) With emotional or cognitive dysfunction. 2. Caregivers: 1. Participating in other studies or have received training in heart failure care; 2. Have left Wuxi within six months.
