18 Years to 70 Years

Inclusion criteria

Inclusion criteria: 1. Aged >= 18 years; 2. Hemodialysis therapy was performed for at least 3 months, and at least 2 per week; 3. Have independent reading and thinking ability, willing to cooperate, able to carry on normal language communication; 4. Clear awareness, knowing their diagnosis, agreeing to participate in the study and signing informed consent; 5. With one or more chronic morbidities from the specified list; 6. The score of Chinese Multimorbidity Treatment Burden Questionnaire was >= 10; 7. The score of Resourcefulness Scale <= 83.

Exclusion criteria

Exclusion criteria: 1. Unstable condition, have had or plan to have major surgery within the nearly 1 month; 2. Peritoneal dialysis; 3. Renal transplantation were also performed; 4. Also participate in other clinical trials.
