60 Years to 90 Years

Inclusion criteria

Inclusion criteria: 1. Aged >= 60 years and >= 2 chronic diseases; 2. Taking >= 5 drugs for >= 3 months at the same time; 3. Be self-aware and willing to cooperate with medication guidance; 4. Sign informed consent.

Exclusion criteria

Exclusion criteria: 1. Elderly patients with malignant tumors; 2. Survival time of patients <= 3 months; 3. Patients with mental disorders or cognitive disabilities.
