Inclusion criteria

Inclusion criteria: 1. Diagnosis with hypertension, diabetes or coronary heart disease; 2. Having ability to operate computer or mobile phone; 3. Living in Xi'an city; 4. Voluntary participating.

Exclusion criteria

Exclusion criteria: Patients with documented cognitive impairments or severe medical complications.
