Inclusion criteria: ?Patients =65yr ?Following orthopedic surgery admitted to ICU ?NRS score during rest=4

Exclusion criteria

Exclusion criteria: ?Allergic to any of butorphanol, remifentanil or dexmedetomidine ?Mechanical ventilation ?Severe tachycardia or bradycardia ?Vasopressors use ?Psychosis ?Alzheimer's Disease
