Inclusion criteria

Inclusion criteria: (i) permanent residents of the community aged 60 years and above who will be living in the community for the next 6 months; (ii) mild to moderate dependence (score 3-7) as rated by the Instrumental Activities of Daily Living Scale (IADL); (iii) agreement at baseline to use the self-management system developed in the pre-study phase of this study and to develop self-management behaviours; and (iv) informed consent and signing of the informed consent form.

Exclusion criteria

Exclusion criteria: (i) Significantly significant illness, such as malignant cancer, severe cardiovascular disease, or need for major organ transplantation that affects self-management behaviours and follow-up; (ii) Severely impaired in any of the five dimensions of intrinsic ability, including MMSE score less than or equal to 9 in the cognitive domain, GDS-30 score greater than or equal to 21 in the depression domain, SPPB score less than 7 in the mobility function domain, MAN-SF score less than 8 in the nutrition MAN-SF score less than 8 in the domain of nutrition, HHIE-S score greater than or equal to 24 in the domain of hearing, and best corrected visual acuity less than 0.3 in the eye with better vision as measured by Landolt C in the domain of vision; (iii) participating in other health-related interventions in pilot studies.
