Inclusion criteria

Inclusion criteria: Inclusion criteria for patients: 1. Aged >= 60 years; 2. Meet the diagnostic criteria for comorbidities; 3. The Chinese version of the Tilburg Frailty Assessment Scale score >= 9 points; 4. Have basic listening, speaking, reading, and writing skills Ability to communicate effectively; 5. Informed consent; 6. Living in Nantong urban area for >= 6 months. Inclusion criteria for primary caregivers: 1. Responsible for the main care work (spouse, children), if there are multiple caregivers, the average weekly care time is >= 5 days, and one caregiver who takes care of the patient for >=4 hours per day is the main caregiver Caregivers; 2. Have basic listening, speaking, reading, and writing skills, and be able to communicate effectively; 3. Informed consent; 4. Live in Nantong urban area for >=6 months.

Exclusion criteria

Exclusion criteria: Exclusion criteria for patients: 1. Severe cognitive, hearing, visual impairment, etc.; 2. Patients with severe physical and psychological diseases requiring intensive care; 3. Those with a history of emotional disorders, brain tumors, and severe brain trauma; 4. Drug abusers; 5. Have participated in or are participating in similar research. Exclusion criteria for primary caregivers: 1. Those with speech and communication impairments; 2. Those with mental or organic diseases; 3. Paid persons such as nanny care workers; 4. Those who have participated in or are participating in similar Research.
