Inclusion criteria
Inclusion criteria: The patient: 1. suffers from one or more incurable chronic diseases, and the independent predicted survival time of the doctor in charge and another doctor with a senior professional title is 1-6 months; physical activity function assessment (Karnofsky Scale) <= 50 points 2. 60 years old or above; 3. Understand the diagnosis and treatment of diseases; 4. Ability to express oneself; 5. Understand and voluntarily participate in this research. Family members: 1. family members or close relatives of the patient; 2. undertake the main responsibility for taking care of the patient; 3. Being able to express oneself; 4. Understand and voluntarily participate in this research.
Exclusion criteria
Exclusion criteria: 1. Those who are in critical condition; 2. Cognitive or communication impairment; 3. taking psychotropic drugs; 4. Received other psychotherapy within the past 3 months Family members: 1. have cognitive or communication impairments; 2. taking psychotropic drugs; 3. Received other psychotherapy within the past 3 months.
