Inclusion criteria
Inclusion criteria: (1) aged 45 to 80; (2) suffering from more than two NCDs with hypertension which was diagnosed in conformity with the standard of primary hypertension specified in the Guidance for Hypertension Prevention and Control Management at the National Grassroots Level (Version 2020); (3) used more than five drugs; (4) with stable health statuses; (5) provided informed consent to participate in this survey and were conscious and could communicate effectively.
Exclusion criteria
Exclusion criteria: (1) with psychological and mental illnesses or other mental health conditions requiring medication; (2) with critical illnesses such as severe hepatic impairment, renal insufficiency, and malignant tumors, who were unable to withstand the process and effects of the evaluation; (3) could not be contacted during the first telephone follow-up and had no other contact information. Patients who died or without complete clinical data were also excluded.
