Inclusion criteria
Inclusion criteria: 1. Adult patients aged>=18 years old; 2. Those who have not previously suffered from heart failure, have been diagnosed with heart failure, or have been repeatedly hospitalized due to heart failure; 3. During hospitalization, the main medical diagnoses clearly include patients with acute and chronic heart failure (ICD-10 disease code: 402. x1404. x1, 404. x3, 428. xx), as well as various types of chronic heart failure patients; 4. Hospitalization diagnosis and treatment information can be fully obtained (including EHRs such as medical and nursing records); 5. Patients who are re admitted to the hospital due to heart failure regardless of the underlying cause; 6. Those who are re admitted to our hospital or have received treatment at other medical institutions and can clearly identify heart failure as the cause (such as providing disease diagnosis certificate, first course record, discharge summary, etc.); 7. The interval between two hospitalizations is within 30 days, including those who are discharged on the day of discharge and those who are re admitted on the 30th day after discharge; 8. For patients who have been hospitalized multiple times (>2 times) due to heart failure within 30 days, the first hospitalization information shall prevail; 9. Informed consent and voluntary participation in research.
Exclusion criteria
Exclusion criteria: 1. The previous hospitalization outcome was automatic discharge, transfer, or death; 2. Hospitalization to other departments or medical institutions due to non heart failure reasons during the interval between two hospitalization treatments for heart failure; 3. Refuse to accept follow-up, loss of follow-up, or withdraw from the study midway; 4. Patients with missing data in medical records exceeding 20%.
