Inclusion criteria
Inclusion criteria: 1. Patients diagnosed with type 2 diabetes according to WHO diabetes diagnostic criteria in 1999; 2. Aged between 18 and 80; 3. Recent HbA1c>=7.0%; 4. Patients with communication ability, no obvious cognitive impairment and able to use smart phones; 5. Patients who are willing to cooperate with long-term follow-up and return visit on time; 6. Understand the research process, agree to participate in the research project, and sign the informed consent.
Exclusion criteria
Exclusion criteria: 1. Any patients without type 2 diabetes, such as T1DM, gestational diabetes, etc.; 2. Patients who have difficulty in verbal communication and cannot communicate well with clinical pharmacists; 3. Patients with poor compliance and resistance to examination; 4. Severe cognitive impairment, lack of self-care ability; 5. Patients participating in other clinical trials at the same time; It also includes circumstances other than those mentioned above that are not suitable for inclusion in the study.
