control group:Routine post-discharge follow-up and guidance
intervention group:Provide remote diagnosis and treatment and rehabilitation guidance for discharged home patients based on stroke remote management system

Inclusion criteria
Inclusion criteria: 1. Diagnosed with stroke (in line with the revised diagnostic criteria of the "China Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke 2018", stroke patients diagnosed by cranial MRI and/or CT examination); 2. The upper limb of one limb was present at the time of discharge Or (and) lower limb muscle strength = grade 4; 3. Consciousness, cognitive function AMT score (Abbreviated Mental Test) = 6 points; 4. Stroke onset = 3 months; 5. Patients or (and) caregivers will use Smartphones; 6. The patients and their families were informed and voluntarily signed the informed consent form to cooperate with the completion of this study
Exclusion criteria
Exclusion criteria: 1. Those with severe heart and lung, liver, kidney insufficiency or other malignant diseases; 2. Those with a history of depression, dementia or other mental illnesses
