Interventions

Control group:The control group was induced by fentanyl and maintained by remifentanil.
Experimental group:The experimental group used esketamine hydrochloride for induction and maintenance.

Inclusion criteria

Inclusion criteria: 1. Age >= 60 years old; 2. Those who plan to undergo elective lower extremity joint (hip or knee) replacement surgery; 3. ASA I-III;; 4. Be willing to sign the informed consent.

Exclusion criteria

Exclusion criteria: 1. Patients with long-term chronic pain (lasting more than one month); 2. Long-term history of opioid use (daily use of opioids > 3 months), or taking sedatives; 3. People with infectious diseases and blood system diseases; 4. Multiple injuries, trauma patients; 5. Use nerve block and other methods for postoperative analgesia during the operation; 6. BMI > 35 kg/m2; 7. Unable to use MMSE or NRS scales; 8. Patients with severe disturbance of consciousness or mental system diseases (schizophrenia, mania, bipolar abnormalities, insanity, etc.) and cognitive dysfunction; 9. Have a history of serious cardiovascular disease (congestive heart failure, severe angina pectoris, or unstable angina or myocardial infarction within 6 months), severe pulmonary hypertension; 10. Patients who are known to be allergic to the active ingredients or excipients of esketamine hydrochloride injection or patients who are contraindicated in the use of esketamine hydrochloride injection: (1) Patients with serious risk of elevated blood pressure or intracranial pressure; (2) Patients with high intraocular pressure (glaucoma) or penetrating ocular trauma; (3) Patients with poorly controlled or untreated hypertension (arterial hypertension, resting systolic blood pressure exceeding 180 mmHg, or resting diastolic blood pressure exceeding 100 mmHg); (4) Patients with untreated or undertreated hyperthyroidism (hyperthyroidism).
